Pre-Application Process:

1. Complete and submit the Pre-Application form below.
2. After the Pre-Application form is submitted you will be redirected to the Criminal History Form.
3. Upon return of favorable criminal history background check(s), your family will be scheduled an appointment for completion of the full application
